All services can be reoccurring or scheduled as-needed. A mileage fee will be applied for any transportation services rendered by J.A.P Senior Services, LLC.
Good Morning To You!
A spirited morning welcome visit that helps you get ready for the day! This includes assistance with your morning routine of dressing and grooming, making the bed, a friendly medication reminder to keep you on track, and an escort to and from breakfast
Welcome Home
Assistance with unpacking, cleaning, and adjusting to their new residence.
Up And At 'Em
All that is included in the "Good Morning To You" Package in addition to a shower and shampoo
Sweet Dreams
An evening visit to get you ready for a good night's sleep. A light snack, nightwear assistance, teeth brushing, medication reminder, turn down and tuck in service.
Discharge Assistance
This service provides "peace of mind" when transferring from a hospital or rehabilitation setting to home/independent living community. Service includes transportation home, discharge planning with nurse, and getting prescriptions filled.
Room And Board
A cozy private or shared room in one our J.A.P homes. This can also be combined with any of our services.
Consultation
A scheduled consultation with our on-staff Registered Nurse and/or social worker.
Grocery And Meal Preparation
Assitance with grocery shopping and meal preperation.
